Referrals

If your plan requires you to obtain a referral from your primary care physician before coming to our office for treatment, be sure that your primary care physician knows all of your pulmonary complaints so that he or she can fully complete the referral form. We are authorized to treat only those conditions listed on the referral form. Without the appropriate referral form signed by your primary care                 physician, we are not authorized to treat you. Our office staff will be happy to assist you in obtaining the appropriate referrals.

 

 

Fee and Payment

We expect payment at the time of your appointment. We will file your insurance claim directly for these services. Our patient accounts staff, can answer any questions you might have. If you have any questions about your plan, please contact your insurance carrier. Please note that this brochure is provided as an service to patients and that your managed care plan can change any of these                  requirements.

 

 

Non Covered Services

Some pulmonary specialty services may not be covered by your managed care plan. If you would still like to receive these services, you will be responsible for the entire cost. Our staff will be happy to   discuss fees for non-covered services with you.

Cancellation

Outpatient appointments are made on a limited  basis on referral by your physician. Please notify us as early as possible if you are unable to make your scheduled appointment. cost. My office staff or I will be happy to discuss fees for  non-covered services with you.
