We would appreciate it if you would complete the enclosed medical information sheet. Please bring all your medicines. If  you have had a previous PFT, chest x-ray within the last three (3) months,  CT Scan of chest and medical records, we would appreciate your bringing them with you.

 

 

Medical insurance

Please bring your insurance card to this appointment so we can file your insurance for you for treatment done in your evaluation. Your insurance reimbursement may not cover the full cost of your studies or physician services. Regardless of insurance, payment remains your personal responsibility.
